Payment Options for Medical Assistance Claims/Probate/Estate Recovery:

Please include the name of the person that received Medical Assistance (MA), their MAXIS case number, and your email address in order to correctly match the payment to the correct account. If you do not have the MAXIS case number, please include the date of birth and last 4 digits of the social security number of the person that received Medical Assistance (MA) in place of the case number.

Payment can be made by eCheck or credit card. There is a $1 fee for using eCheck or a 2.5% fee for using a credit card. The fee is applied to the eCheck account or the credit card depending on the payment type selected.

Payment by mail: 

Please make checks payable to the Anoka County Human Service Division and mail to:

Anoka County Economic Assistance Dept., Blaine Human Services Center, 1201 89th Ave NE, STE 400, Blaine, MN 55434-3373.

Please include the case number or the person that received Medical Assistance or the person's date of birth and last 4 digits of their social security number.
