2025-2026 Community Partner Awards 

We’re excited to announce that the Anoka County Statewide Health Improvement Partnership (SHIP) is now accepting applications for the 2025–2026 Community Partner Awards! 

The Community Partner Awards are designed to support local organizations in advancing policy, systems, and environmental (PSE) changes that promote health and equity throughout Anoka County. Whether you’re working to improve access to healthy food, expand opportunities for physical activity, create commercial tobacco-free spaces, or support mental well-being—this is your opportunity to make a lasting impact in the communities you serve. 

Applications are open to schools, workplaces, community organizations, and others looking to champion projects that reduce health disparities and improve well-being for all residents—especially those most impacted by structural barriers to health.
