   The form below should be used to upload the supporting documentation for a previously submitted Solid Waste Management Charge appeal application. Any other reason for appeal or if you have questions about the Solid Waste Management Charge call 763-324-3481.

    Information about the garbage and/or recycling service(s) at this property must be submitted in order for application to be complete.
20. Provide one of the following for each garbage and/or recycling hauler or individual that removes garbage and/or recycling from this parcel:
    1) A signed letter from the garbage hauler/individual stating the volume of garbage and/or recycling collected from the parcel and the frequency of pick-up in the past 12 month, often called a "Summary of Service". DO NOT specify tonnage unless the hauler charges based on weight. or 2) Copies of your garbage and/or bills from the past 12 months, showing the size of the container(s) and the frequency of pick-up. or 3) A signed letter from company/organization representative stating the volume of garbage and/or recycling collected from the parcel and the frequency of pick-up in the past 12 month. *** For recycling service, please indicate the types of materials recycled.
